Policies which the nominator knows the nominee supports or in which they have expertise:

Dr. Vinay Prasad, MD, is an American hematologist-oncologist and a professor known for his critical analysis of medical practices, health policies, and drug regulations. He often emphasizes evidence-based medicine and is a vocal advocate for critical assessment of clinical trials and regulatory processes in healthcare. Here are some specific areas of policy and practice he often addresses:

    1. Drug Approval and Regulation: Dr. Prasad has been critical of the U.S. Food and Drug Administration (FDA) for approving drugs based on limited or surrogate endpoints rather than strong clinical evidence. He advocates for higher standards in clinical trials and for drug efficacy and safety to be thoroughly proven before market release.
    2. Evidence-Based Medicine: Prasad champions the use of randomized controlled trials (RCTs) as the gold standard for proving medical treatments’ efficacy. He has critiqued the use of observational studies in place of RCTs, especially when they influence significant treatment decisions.
    3. Cancer Care and Screening: He often questions the overuse of cancer screenings and treatments that lack clear benefit. Dr. Prasad has highlighted that certain cancer screenings may lead to overdiagnosis and overtreatment, potentially causing harm without improving survival rates.
    4. Healthcare Costs and Ethics: Prasad has discussed the ethical implications of high drug costs and believes that expensive treatments should be justified by substantial benefits. He supports transparency in healthcare pricing and advocates for ethical pricing practices, especially for life-saving medications.
    5. COVID-19 Policies: Dr. Prasad was an outspoken critic of certain COVID-19 policies, including prolonged school closures and strict mask mandates in children. He argued for a more nuanced approach to risk assessment, especially in areas where he felt restrictions were based on weak evidence.
    6. Conflicts of Interest in Medicine: Dr. Prasad has highlighted issues of potential conflicts of interest in medical research and practice, particularly involving pharmaceutical company influence on clinical guidelines and physician recommendations.

Dr. Prasad’s policy positions are generally aimed at promoting rigorous scientific standards, transparency, and cost-effectiveness in healthcare to ensure that treatments are genuinely beneficial for patients.

Nominator's thoughts on what would make this nominee a valuable member of a future Trump Unity Government

Dr. Vinay Prasad, MD, demonstrates these characteristics through his work, public speaking, and published research. Here’s how he exemplifies each:

    1) Is Competent

    • Background and Expertise: Dr. Prasad is a hematologist-oncologist and professor with a focus on evidence-based medicine and health policy. His extensive background in both clinical practice and academia shows a high level of competence in his field.
    • Published Research and Books: He has authored influential research papers and books such as Ending Medical Reversal, which critiques and calls for improvement in medical practices based on weak or outdated evidence. His publications are well-regarded in academic circles, demonstrating his expertise and commitment to high standards in medical science.

    2) Is Honest

    • Transparency in Critiques: Dr. Prasad’s critiques of the FDA, pharmaceutical companies, and the medical establishment are based on evidence and openly published, often including detailed analyses of clinical trial data and regulatory decisions.
    • Acknowledgment of Complexity: He does not oversimplify issues, openly discussing both the strengths and weaknesses of different perspectives. For instance, while critiquing drug approvals, he acknowledges the challenges regulators face and doesn’t present solutions as easy fixes, reflecting honesty about the complexity of healthcare.

    3) Is Respectful

    • Engagement with Opposing Views: Dr. Prasad often engages in debates with professionals who hold differing views, maintaining a respectful tone. While his critiques are direct, he emphasizes that his focus is on improving medical practices, not on personal disagreements.
    • Constructive Criticism: Even when challenging widely accepted practices or regulations, Dr. Prasad avoids ad hominem attacks and instead focuses on data and evidence. He frequently collaborates with others in the field, showing his respect for colleagues even when he disagrees with them.

    4) Has Integrity

    • Ethical Stand Against Conflicts of Interest: Dr. Prasad is outspoken against conflicts of interest in medicine, advocating for transparency and accountability when it comes to pharmaceutical company influence on medical research and practice.
    • Consistent Advocacy for Evidence-Based Medicine: His work consistently advocates for high standards in research and patient care. He doesn’t alter his stance depending on popularity or external pressure, showing a commitment to integrity in all his public statements and publications.

    5) Has Courage

    • Critiques in High-Stakes Contexts: Dr. Prasad has challenged powerful entities in the healthcare industry, such as pharmaceutical companies and regulatory bodies like the FDA. These critiques, especially regarding drug approvals and conflicts of interest, require courage given the influence and resources these organizations hold.
    • Stands Taken During the COVID-19 Pandemic: He voiced critiques of certain COVID-19 policies, including school closures and mask mandates for children, during a time when public sentiment and medical authority were highly polarized. Despite backlash, he continued to advocate for policies he believed would benefit public health in the long term.

    6) Has a Proven Record

    • Impactful Publications: Dr. Prasad has a substantial body of published research that has influenced discussions around evidence-based medicine and medical ethics. His book Ending Medical Reversal has been widely referenced and praised for its insights into improving healthcare.

His work has contributed to broader awareness of these issues and encouraged other professionals to re-evaluate established practices.

Dr. Prasad’s career reflects a blend of expertise, ethical commitment, and courage, solidifying his role as a thought leader in healthcare policy and medical integrity.

  • Great choice. I came here to add Dr Prasad as well. His advocacy for honest, solid science and evidence-based medicine matches our goals with MAHA.

Excellent choice. I agree that Dr. Prasad’s expertise in evidence based medicine and the courage he has demonstrated during the past four years by speaking out against the pharmaceutical industry, the FDA, the CDC, the Biden/Harris administration, and corporate capture in the university setting, among other issues, has provided much needed illumination of egregious ethical violations that create harm to the people of our country and the world as a whole.

I’ve been reading Dr. Prasad’s substack for 2 years now, and I appreciate his high standards for what constitutes “proof” and his ability to find the flaws in research and to explain what went wrong to lay-people.

Dr. Prasad is the definition of having real values and holding to them. His books “Malignant” and “Ending Medical Reversal” are great cornerstones to understanding pre-test probability and the ways that we do not hold medicine to the high standards that are needed, particularly when developing policy level decisions. Could not recommend anyone more.

Dr. Vinay Prasad is an effective communicator, an excellent researcher, and is both thoughtful and incisive on a range of public topics. He (literally) wrote the book on how bad policy and bad evidence harms people with cancer. The thing I most admire him for is his capacity to very technically break down the evidence for clinical trial evidence, and, at the same time, to communicate what the actual evidence says and doesn’t say to the broadest possible audience. Those are usually very distinct skills, and very rarely present in the same person. Finally, the quality of Dr. Prasad’s analysis is based on the pursuit of truth and the best available evidence. He is nuanced in a way that has mostly been lost, and follows the evidence to make his conclusions, not in pursuit of professional or persona advancement. I’ve learned more from his writing and podcasts than from my MPH program, and the thought of having a major public health agency under the leadership of someone of his caliber of intellect and integrity would not just be hopeful, but would do what I thought was impossible and help to remove some of the cynicism about public health.
