I have tried to submit my request for nominee but incurred a site error. I tried to attach a screenshot of the error message.
Can you please help?

I am experiencing this as well, are there any solutions to successfully submit?
Be patient, 
I think it takes time for it to go through after you submit it
This is an issue that shows up when
• you switch between webpage tabs on your browser a lot while filling out the form
• the webpage times out because you’ve been on the webpage a while and left it sitting while being away from your device for a while (the dog asks for a walk while you’re filling out the form and come back to it)
• your internet connection is spotty, and while you were filling out the form, it disconnected and reconnected before you clicked submit, without you noticing.
Solution:
Take a screenshot of the fill out submission form before filling it out.
Open up a word document of mobile notes document, and put everything there (bio and socials links, contact info, which category you’re going to pick, and your paragraphs in the write in sections).
Refresh your internet browser and make sure your internet connection is good, quickly open up the submission form and quickly fill it out without switching browser tabs and without taking too much time.
The submission should go through without a timeout error.
This is very common on websites with webforms, all webforms have timeout errors because contact information is being filled out, and this is a built-in mechanism to protect privacy and encryption. This is not something they can fix, you have to change what you’re doing while filling out the form.
